Who Can Join?

The program provides health services to children who are under age 19 and their adult caretakers. To qualify, your family cannot make more than certain amounts per year.

How Do You Join?

To get an application, call (888) 557-1116 (TDD/TYY 1-877-6-PARENT) or you can pick up an application at:

• Your local library
• Giant stores
• Safeway stores
• CVS drug stores
• Rite-Aid drug stores
• DC Department of Employment Services
• DC Department of Motor Vehicles
• DC Department of Tax and Revenue
• Any IMA Service Center

After you fill out the application, you can mail it in. Be sure to include proof of income, Social Security number, child care expenses, and proof that you live in DC. Mail the application to:
